One survey per person.

Surveys must be completed by close of business Monday May 14, 2018 to be eligible for the drawing.

One survey submittal will be randomly drawn for a $50 Amazon Gift Card.

Members of the Governor’s Industrial Safety and Health Advisory Board, Construction Planning Committee members, Presenters, Exhibitors, and employees of the Dept. of Labor & industries are not eligible for the drawing.

Notification of winners will be made to the email on the registration. The email will include the gift card redemption instructions.
