Surveys must be completed by close of business Monday October 1, 2018 to be eligible for the drawing.

Members of the Governor’s Industrial Safety and Health Advisory Board, Planning Committees, Presenters, Exhibitors, and employees of the Dept. of Labor & industries are not eligible for the drawing.

Notification of winners will be made to the email on the registration. If you wish to update your email, you will need to use the Modify/Update link in your confirmation email. You may also send an email with your Name, Registration Number and updated email to info@gishab.org. This must be received by close of business Monday October 1.
